After you receive your participant ID card you can create an online account and receive personalized access to your claims and plan specific information.

Coordination of Benefits
As the third party administrator for the Froedtert Health medical plan, we are annually required to perform a Coordination of Benefits (COB) review. The COB review verifies if you or any of your covered family members have additional medical coverage.
A coordination of benefits form must be completed and returned before any claims can be processed. Other insurance may include, but is not limited to, coverage through a spouse’s plan, court ordered insurance coverage, coverage required in a divorce decree or paternity suit, or Medicare. If you provide incorrect or inadequate information, claims may be delayed or denied.
Log in to your account to complete the COB online. You’ll find “Coodination of Benefits Update” in the Quick Links.
